Consultation forms and patch tests before your visit

Some businesses need a little information from you before your appointment.

Fill in a consultation form

   Open Account, then Forms.

   You'll see which forms are due, the business they're for and when they're needed.

   Open a form, answer the questions, sign if asked, and submit.

Patch tests

Some colour or chemical treatments need a recent patch test. If yours is missing or out of date, you won't be able to book that treatment online. Contact the business to arrange a patch test.

Leave a review

After your visit, open Account, then Reviews, to rate the business and write a few words. Reviews from verified visits appear on the business's page.
