Consultation forms collect information from clients before their appointment, such as allergies or medical history.

Create a form

  1. 1
Open Operations → Consultation Forms and create a form. You can do this on the web or in the Cobbtree app.
  2. 2
Add your questions. You can use text, long text, yes/no, select and date fields.
  3. 3
Choose whether the form is required on a client's first visit.
  4. 4
Publish the form so clients can fill it in.

Edit a form

Open the form, add or change questions, and save. New answers use the updated questions.

Review answers

  1. 1
    Open Form Responses.
  2. 2
Filter to the responses that need review and open one.
  3. 3
Read the answers, then select mark reviewed. You can reopen a response later.

To see a client's earlier answers, open their profile, or filter Form Responses by client. Answers stay available after you've reviewed them.

Good to know

Forms can't be attached to individual services yet. If every new client needs to fill one in, use required on first visit.
